Mark sheets of qualifying examinations (10th, 12th, and any previous semesters if applicable)
Transfer Certificate (TC) from the last attended institution
Migration Certificate (for students from boards other than MP Board)
Caste Certificate (for SC/ST/OBC candidates, issued by a competent authority)
Income Certificate (for economically weaker section candidates)
Domicile Certificate (for Madhya Pradesh residents)
JEE Mains/MP PET scorecard (for engineering admissions only)
Passport-size photographs – 4 copies

The originals of these certificates must be presented at the time of admission for verification.
